Do you code dehydration and UTI on a readmission assessment?

Assuming you state is like MN, our payment changes with the Annual, 2nd

your shoes if/when the state catches on to that plan. It certainly
reeks of fraud.

Subject: multiple annual assessments

This question is for case mix states - recently had a consultant say
that it was OK to do annual assessments every 3 months (not sig changes)
instead of quarterlies in order to capture a higher case mix. Does
anyone actually do this? and what are your thoughts on this?
