The psych
hospitalization that started 1/22/04 extended her skilled
period thru the discharge date of the hospital stay. You then have 30
days in which you can restart Med A and it doesn’t matter whether the psych
stay was for 3 days or not. Since today is 2/12/04, 30 days have not
lapsed and she is eligible to use the remaining 92 days of this benefit
period. I would skill her again and bill Medicare. Even
though the Psych hospital stay was for a dx other than the pneumonia, it all
is interrelated and therefore covered.

Subject: Placing
back on MCR after taking off...
We have a resident
whose last MCR day was 01/21/04. On 01/22/04 she was d/c'd to a psych.
hospital. Upon return she was not skilled. Now therapy has picked her up to
treat 5x/week x 2 weeks for ther. ex., gait training... etc. The 3 day stay
that began the benefit period was due to pneumonia (which caused the sickness
that led to the weakness that led to the therapy - I hope that sentence makes
sense). So, can she be skilled again? And if so, is it an
option or a requirement that she be returned to MCR (not that I do not
want to place back on MCR, but I am wondering for if the situation
comes up again). She has approx. 92 days left.
