definitely have the wound nurse stage .  question does your wound sheet have present passed and deepest depth.  this sometimes helps. Also, there should be a place where unstageable is written onthe sheet that way you can get a protocol written that states when wound nurse writes uunstageable it is considered a stage 4 on MDS.
