agree with this, as an addenda to my previous message, I think the big issue
is as noted here, PUT IT ALL IN THE RAP AND CARE PLAN and you've covered all
bases.

There must be variations in interpretation. By history I had always
reflected any variations throughout the 180 days, but at a seminar in early
winter was told we were to capture the picture by look at current vs a
snapshot of what it looked like 180 days ago. You just can't win. I guess
I'm going to try to be apology to the surveyor, begging forgiveness for my
stupidity in coding....and reflect it ALL in the rap.

the last 180 days. My interpretation had always been IN 180 days, so I
compared weights from now and 180 days ago. The surveyor said this was
incorrect and any weight loss of 10% at any time during that period, whether
they recouped the weight or not, should be counted.

resident who has frequent weights (q week/q2 weeks) during the 30 days/180
days, for what ever reason. She is not always taking the current weight and
comparing the weight closest to the 30 day or 180 day to determine a 5%/10%
weight loss. If there was say a 10% weight loss during 180 days with 2
arbitrary weights, but not with the current weight compared to the weight
closest to 180 days - she feels this should be reflected in the MDS.

address that period of weight loss in your notes, but it does not meet the
definition for it to be addresssed in the MDS - I wanted to make sure we
were right before addressing with her.

interested in knowing how everyone fills out section K3 Weight Change. You
are looking back (from the ARD) 30 days and 180 days - you take the current
weight and compare with weights done 30 days ago and 180 days ago - we got
that. The question is if there is more than one weight done in that 30 day
look back - do you compare your most current with the last one done in that
30 day window or do you compare with the lowest weight recorded regardless
if it is closest to the most current or the highest weight in the window.
How are you all doing this?? Thank you in advance.
