Keep in mind that using a "4" means the resident was totally dependent and didn't participate, but the task was totally performed by the staff..Think about dressing for example--if coded a 4, the resident didn't lift an arm or anything to help?? staff did it all. Is the coding correct? I agree with Delores, consider the whole picture of the resident and make a clinical judgement based on the needs of the resident, care plan etc. The RAI is a good guideline, but sometimes you have to do a little "critical thinking" or as I say "think outside the box"
Defending not doing a significant change is much harder than doing one at times of need.
