My facility has just changed the forms the CNA use to document their ADL care. It does not differentiate the type of assistance just whether it is a one person or two person assist and the flowsheet does not montior whether the person is continent or incontinent. I have found that interviewing the CNA and documenting on a "MDS Progress Note" that I made up myself stating "Per interview with CNA" that I have covered myself and feel that my MDS is probably a lot more accurate than if I just went by the nursing notes.
