Culture change is whatever the facility initiates that makes their home resident center and resident choice.  One of the things we are doing is a continental breakfast for residents that do not want to be on the "Get Up" list.

Because you would interview with family members and/or loved ones and find out what the resident enjoyed doing, or

particular "routine" and put it in the careplan In the first person context, (as if the resident themselves is talking to you)

such as:    intervention:  I enjoy taking a nap after lunch around

1pm.  I enjoy working with flowers/plants.  I think you can see the idea.  One of the replies to the post stated that the

surveyors from the state were extremly impressed with these.

We are looking for examples or a temp plate for making care plan specific for our alzheimers units. I remember in the way back past someone mentioning using a story book type care plan to address resident needs strenghts, etc.....
