Thanks Faye, she does not have tray.  I just went down to my DON and told her that we need to get an order for the Geri chair with the reason we use it. To my surprise my DON told me not to write the reason, just: ”up in Geri chair daily”. She said there is no need to put reasons…since it’s not  a restraint; otherwise she said we would have to get orders for w/c too. Do I miss anything here? If so you also need to assess if the tray is restraining by limiting access to ones body, limiting movement (think readjust clothing, scratch knee type of thing).  IF she normally could perform these functions and the tray is creating a barrier then this is still a restraint. Ok, so my questions are:  Is the Geri chair considered to be restraining patient in this case?
