So the question again is if what you are saying here is cover the resident then if a resident had 60 days of unskilled services then started on PT 5x a week on Medicare part B then was hospitalized and came back continuing to need PT 5x a week she could be covered under Medicare Part A, right?

If she had been in your facility "for years" on medicaid and had not recently been on medicare and had not been a hospital inpatient and had not had skilled level of care for 60 consecutive days, the IV's in your facility would not matter. She would have had a break in her spell of illness, probably long ago if she even had any medicare coverage-(doesn't sound like she had skilled services) . The break in the spell of illness in Medicare counts from the end of the last day of coverage or daily skilled services following the discontinuation of Medicare with no skilled services for 60 cosecutive days. At that time a new period begins.
Cover her on Medicare if she fits the criteria.
