I agree. You base whether to project into a low, medium, or high, or whether to use grace days (if needed) to capture a very or ultra based on the needs to each individual resident. There is no way a blanket policy to project will cover everyone. Medicare is likely to frown on this. Some residents will truly need the grace days to get the therapy that they need.
Also, there is nothing wrong with projecting to capture a low, medium, or high if this rugs category fits the resident. There is no point in using grace days to capture these rugs scores if the resident will fall into one of these categories anyway.

We use lots of projections and few grace days. What is important is that the documentation supports the projection and really supports the reasons if the projection is not achieved. Usually our projections have been pretty accurate. You also need to look at each individual resident and not just have a blanket policy that everyone is projected into high, or everyone uses grace days.
