Prior to an MDS assessment during the assessment
period, what tool is used to gather info on ADL'S to use to complete the
MDS? We have CNA'S filling out a functional assessment form but it needs
to be more user friendly; if anyone is willing to share it would be greatly
appreciated. Thanks!
