I would definitely code the problem, and (as you stated) make sure to document all conversations about the recommendations.

Question for the group.  I have a resident that has swallowing problems from a CVA, not new, she was on thickened liquids in the past but her or her daughter do not like.  So she no longer gets thickened liquids.  She has choking episodes when she drinks, my question is.  Should I mark swallow problems on the MDS?  I always make note that her and daughter refuse to have the thickened liquids even after education.
