i also developed a tracking tools and i take it to stand up and note
possible sig changes, i review the census for changes and i check the change
of condition book daily for sig changes, the nurses have been educated to
let me know when they think they have a condition change according to the
guidelines this process went through survey with out any problems and i have
not missed any condition changes, the nurse assistants had inservices on
recognizing and reporting condition changes to the mds coordinator

They will probably want to see a prevention strategy-- a plan to prevent it
from happening in the future.  Attempt to figure out why it was missed and
then go from there.  We had missed one also, a few years ago on survey.  The
reason was that one of the MDS coordinators went off on leave, had made a
note to monitor this resident for a change, and no one knew about it.  From
then on, we have had a tracking tool that we write any resident on that
looks as if they may be having a change.  Then we write whether we proceeded
with significant change or not, and make notes accordingly in the chart.
This way, hopefully no one will get missed.  Just an idea.  Good luck.

surveyor found a resident that should have had a significant change done
several months ago.  Agreed that we missed it and then stated we will
complete one tomorrow.  Anything else we need to do?
