Early in PPS, some large chains told their nurses to use a certain day for all Medicare 5 day assessments. Some told them to always use day 5. Some even told them to always used day 8. Any policy of this type is going to cause problems both from a financial and regulatory perspective. The bottom line is you can use day 1 - 5 as you wish. If you use day 6-8 you need to have some reasonable justifiation for doing so. The most common justification is that using grace days gives a more accurate representation of the care being delivered to the resident than using days 1-5. . I am sure there are other good reasons for useing grace days, but that is the most common.
