Our facilities use a form that is given to the MDS Coordinator for entry. We have found this to be the best system for several reasons.

I work very closely with the rehab department and they have always put their
own information in the computer and sign for it.We haven't had any problems.

We had problems with therapy putting 14-day information in before the 5-day
MDS was closed so we use a worksheet and nursing puts the info in the
computer.

We did it for about 6 months, and then went back to them giving me a form.
It didn't work for me, therapy was always late.

We are going to be having our therapists complete the minutes on section P
and section T. Can I get feedback from those of you that have therapy fill
out parts of the MDS and how it is working? Thanks. Deb
