It is important to check the actual HCPCS code with the consolidated billing Helpfile to be certain. 

While many chemotherapeutic agents are excluded from consolidated billing, Lupron technically is a hormone and not a chemotherapeutic agent.  It may not be excluded.  That is the same issue that arises with antiemetics that often are given in conjunction with chemotherapeutic agents or radiation therapy.  The chemo or radiation tx may be excluded (again, depending on the code), but the other meds generally are not.

Chemotherapeutic agents should be billed by the licensed healthcare
provider who administers it. It is NOT included in the Med A consolidated
billing rate. (Can't find the exact program memorandum which notes
this...)
