So, is the rule that every medication have an indication for use state-specific? I thought that everyone had to do this.

If the physician wrote (faxed) the order that the antibiotic was for treatment of UTI, but did not include the diagnosis in any other documentation,  would it qualify as a diagnosis necessary for MDS coding?
