In response to the number of requests, attached is a sample of some rap summaries for one particular resident (name changed). Prior to her hospitalization, she lived in an apartment and had several community services active (Meals on Wheels, Visiting Nurse) and she also had a private pay nurse aide 7 days a week. She was fairly independent and could do quite a bit for herself, walked with a rolling walker, and needed minimal assistance with some ADLs.
I could have elaborated more on the Psychosocial Rap note to include more details such as above, but did include her strengths and prior abilities on the POC itself.
