If the resident is no longer skilled and you do not do an OMRA, should a Sig. change be done to re-set the RUG reimbursement rate? If no sig change, is it fraudulent to allow the reimbursement rate to remain at a RM w/o therapy for the remainder of the 60 days?

It should be done on day 8,9,or 10 after completion of therapy.
Not if the resident is no longer skilled.
