4's"? (Actually, I want to ask that regardless of who you were
responding to.)
As I described in my earlier email about this
question, there are certainly situations in which locomotion off the unit
would only occur very infrequently, but that when it does occur, you have to
code it according to the instructions.
I would definitely address in the care plan (and
RAPs if this is a full assessment) why the resident goes off the unit so
infrequently. Maybe it's the resident's choice. Maybe s/he has been ill
recently. Maybe staff needs education and motivation to get these folks
up and moving. Maybe s/he is depressed. You get my meaning.
I, too, would like to see some other responses, but if it were my resident, I
would stand by my earlier response. Even if it only happened once in the week,
if the resident required limited assistance for that one instance, then I
would code it 2/2.

This was my original thought but then I coded
a 1/2 because physical assist only occurred once as in the definition of
supervision. However are the 8/8's considered as 4's in which case a 3/2
would be appropriate. As you can see this is why I am confused & to top
it off our UMR team said it should be 8/8 because the activity didn't occur
3 times. Thanks again. More opinions would be helpful. I think the current
guidelines don't really address this situation clearly.
