So then, would the policies for nursing/CNA read something like this?

It is the policy of this facility to provide nursing care in accordance with accepted professional standards. The procedures in Barnum and Bailey's Manual of Nursing Practices and Procedures  (or whatever real book we select) will be used for education and reference.

Save yourself the time and money.  Don't write a policy to be updated in two years.  Buy a book on good practices, use it as a "reference guide" for educating your staff.
