The PERTS form is our version of an Incident and Accident report we complete for our 
Risk Mgmt Dept.  Some reasons to complete the form would be falls, med errors, 
equipment failure, etc.

We use the point system for each med error and counsel according to point grid.  We 
also use the cumulative method w/subsequent errors which is reviewed on an annual 
basis at time of performance review.
