Facilities no longer have a portion of their historical costs used to calculate the rate- that went away in the 4th year of PPS- everyone is now on the Federal rates with the geographic adjustment.
 
Check your FI's website for a rate calculator- if you can not find one there- then try

about 1/2 way down the page you will find a SNF rate calculator effective 10/1/2003

The rates listed in the Federal Register are the PPS rates, then each facility's 1995 cost report is used to calculate the facility's individual rates.  Your billing office should be able to tell you the specific RUGS rates for your facility.  If they can't, your fiscal intermediary can help you.
