If a resident had a significant weight loss and no other areas of decline would you schedule a significant change assessment(or do you need 2 areas of decline)? If there really isn't any other change, I would update the nutrition care plan and not proceed to a significant change. Usually there are other problems that go along with a significant weight loss unless it was a planned loss.
