We have weekly careplan meetings (we are hospital based SNF) and the decision to keep or discharge is made at this meeting with all disciplines interjecting their rationale for continuing on Medicare or not. It really helps to hear the reason disciplines are, or are not continuing to skill the resident.

Our Medicare nurse keeps them for approx. 2 weeks and then discharges them. We have a new Social Worker who has been in LTC for sometime. She is trying to tell the Medicare nurse that you can gradually take residents off rehab then they will be covered through nursing. I hope this makes sense to all of you.
