Our rehab dept under part B writes orders for 3x a week,this is not skilled[not a 
daily service]I begin my count for 60 day after part A ends,then look for any hospital 
stays.If there is none I start new spell.Did have interesting thing happen.Admitted 
patient under A ,she was in snf end of 2003,used 76 days,went home with gtube,fell out 
of bed on day 61 and admitted into acute care.Seemed to us,she had new spell.CWF 
showed only snf days last year,I didn't see CWF but administrator tells me it only 
showed the snf days from last year.We billed for last month and by the time we got the 
check,we were into co-pay days.Business office found out from medicare she had 1 
overnight on Jan 4th[no 60 day spell]Her husband didn't remember this episode.We found 
this out about 14 days after her benefits exhausted.There is no foolproof system.We 
really can't bill because we didn't give a timely notice.I believe she has an extended 
care policy that pays at 80% on day 101 which I believe we will bill under until we 
gave our notice this week.
