The pressure ulcer has been resolved via the flap, so it is no longer coded as a pressure ulcer.  It would be a surgical site until healed.  The definition of Surgical Wounds on page 3-166 includes skin grafts.  Surgical wounds are not included in M1 and therefore are not staged.

I have a resident that received a flap for a stage iv pressure ulcer. Now that she has returned from the hospital, I am unclear how to code this wound for the mds.  Is it now surgical?  Do I stage it?  Do I code it as a pressure ulcer?
