Unfortunately I can't respond to the QI question but I will give my opinion on the res with the recent CVA. You have 14 days to identify a significant change and I would use those at least 7 of those 14 days to have your res in an assessment period and then proceed with the significant change. This will give you your 7 day look back period to accurately reflect the change in the resident and who knows maybe she will wake up some.
