I know this has been discussed before, but I just can't find it.
Can someone tell me where to find in print about completing MDS's from a chart after a
discharge, say if insurance turned out to be Medicare but pt. admitted under something
else? I know we aren't supposed to go back and do a 5 day, but I need to find it in
writing.
