The 700 and 701 forms were required at one time for Part B.  They are no longer required.  Therapists have taken them and altered them.  My concern is that the original forms included the physician certification and recertification.  If this has been removed please be sure you obtain a separate Medicare Part B Cert/recert form for Therapy.

Our therapy department has their own form which resembles the old HCFA 700 form and they created an update form that covers two weeks at a time, with only space for actual functional status and a spce on the bottom for a brief narrative note. My experience is that the rehab department prefers to use their own forms and are not always real accepting of outside help. You could document progress and issues what ever you use for Utilization Review meetings
