I have a pt who has a midline incision that dehisced.  She had a wound vac
on with us changing twice a week and going to wound clinic for the third
change.  She also was recieving skilled therapy.  

The wound began to tunnel and open up more on the outside.  She went to
hospital and has had several surgical procedures.  They have not closed it.
She will be coming back to use probably on Monday.  

My question is if therapy is contraindicated and after an appropriate
observation period, the only skillable service would be the wound care.  We
would consider this a surgical wound and at least 1 skin treatment being the
three times a week dressing change.  Can I skill on this since the dressings
won't be daily changes.  Would it be enough to be monitoring healing,
infection, promoting nutritional intake?
