State surveyor made a comment on one of our resident's RAP summary. Am I allowed to revised my RAPS according to what the state surveyor wants to see in one of our residents or should I leave it alone? The RAPS tell where we found our information, gives a picture of the
resident and an opinion on why or why not we would take the problem to care
plan. There is nothing that says HOW we have to write them or WHAT needs to
be in them. If the state surveyor doesn't like it, then it's his/her problem. I
know of a few state surveyors who had to write RAPS and they agreed with me on
this one. Leave it alone.
