Delayed therapy

Resident admitted on Med A from hospital post fracture.  Due to slow healing has not had advance in weight bearing status so is being temporarily cut from therapy.  Has follow up visit with MD in 3 weeks which would be in 30 day window but is there any way to write d/c order to allow for resumption of therapy after 30 day window if she doesn’t get increase in weight bearing at that visit. I realize it is to be somewhat predictable at admission but this is an unexpected complication so does this make a difference.  I found the regs but am still not sure. Thanx Faye
