If , indeed , the MDSs were done incorrectly and you can find documentation to support your claim, you can complete a correction form and then either a Significant Change or Significant Correction assessment (depending on which is most correct to complete).  Be aware, though, that depending on your state's case mix rules, it might not be possible to correct the case mix payment retroactively.  It should help the present time though.  It is right to correct assessments that are in error whether the state is case mix or not.  Just be sure you have backup documentation to show clearly that the previous assessments were incorrect.

I recently started a new position as MDS Coordinator.  Case Mix State. 
 
While completing the current MDS I have found that several of the prior assessment were coded inaccurately and we got lower RUG levels.  Can I do significant corrections of prior assessments to get an accurate RUG level for the patient?  I find that major information was not coded so how can the care plan be correct?  (diseases, conditions and true adl levels)
Any guidance will be appreciated.
