I am in a 40 bed unit in a skilled nursing unit in a hospital based snf. Our acuity is much higher than when I began here nine years ago - we do have admit criteria but it seems once the patient is here and they decline you can not get the physician to send them back to acute care hospital - since we are in the hospital the physicians seem to view us as the same as the acute care units.
