We do a daily care plan sheet, it is a short version of the care plan.  Then the aids have a sheet, the old flow sheet they initial.  At the top is the if diabetic meal intake.  At the bottom is a statement they initial basiacally stating they have read and performed the cares as stated on the daily care plan sheet.  I up date them every 92 days, and when something changes with the res.  The ploblem is it's hard to keep current at times with changes between care plans, and wish we had contince pattern.
