I don't think you need to worry about it if her diagnosis would support a very high.  Just because she isn't able to tolerate it doesn't mean you would have known that up front.  Also, if she's not tolerating it, therapy should have notes to support this.

If a resident was given 120 minutes of combined therapy daily x 3 days then 30 minutes, skip 1 days then 120 minutes = 6 days for the 5day ARD (RV) then day 7 decreased to 60-65 minutes daily(RH) because they could not tolerate a higher level and have maintained the RH level, as long as the intolerance is documented-can this be misconstrued as using a grace day to gain a higher level? Therapy swears, they thought the resident could tolerate a higher level.
