She would have had to have had the 5 day Medicare assessment done both
times, but if she wasn't in your facility for 14 days straight you might
not have had the admission assessment done.

On a resident who is admitted to facility, has a 5 day completed, goes
out for 2 days, returns, has her second 5 day and then is discharged to
another facility.  We did discharge prior to initial.  On her first
stay, she was here for 8 days, on the second stay she was here for 8
days.  Should an admission have been completed?  I was going off of my
PPS schedule and days, and looked past the fact that she had previous
days.
