Thank you all for answering. Some missing mds's
were as simple as dc's not being sent. Others were d/t spelling (adding
periods, spaces, etc). Others we still don't know why, but have filled
out the merge forms and sent them to Tallahassee

Double check your transmittal and verification reports to be sure the
assessments were accepted. If they WERE accepted, call your state help desk and
explain what you have to them and ask what they suggest. Most of mine were
actually due to incorrect reasons for DISCHARGE being selected. If that is the
case, submit corrected discharges and that should clear the list. That's what
(illinois) helpdesk had me do and I now have no missing assessments. Good luck,

Subject: missing MDS report
The state surveyer who comes in to do the quarterly
visits asked me to pull up the missing MDS report, and that since we have a
large facility (180 beds) there should be no more than 12 names on it.
Well, there are 25 on it. Dating back to 2001. She wants them
cleared off, but HOW? The risk manager and I don't know how. We
can't figure out why most of the people are on it because our records show the
MDSs were submitted/accepted. Can anyone help me?
