If the therapist enters the minutes in Section P, then the therapist must attest to the accuracy of that item in AA9.  If it is the MDS nurse that enters the minutes in Section P based on therapy documentation in the medical record, then the MDS nurse must attest to the accuracy of that item at AA9.  In that case, the therapist is not required to sign the attestation at AA9.  A close reading of the actual attestation statement will reveal the reason that this is appropriate.

Rena:   Do therapists still have to sign off for their minutes (section P)?   Our center seems to think so, but the recent RAI update (April) deleted that requirement...
