We incorporated our Resident education on the bottom of our
progress notes for all disciplines. We do a Learning Needs assessment on all our
admissions - which identifies the needs and learning styles and hinders to
learning (required by Joint Commission) and then document the actual education
in the progress notes. If you are interested I can FAX you a copy of
it.

Does
anyone have a good,easy to understand education sheet for resident and family
teaching documentation such as a resident with a new peg tube. We would educate
both the resident and the family on it's use. We are currently using one form
our hospital that is really not for long term care and is quite cumbersome.
