We have weekly TILE/restorative Care meetings , PT/OT present as well as administrator, DON and TILE Nurse. Before meeting MESAVS are printed out and 21i's are reassessed. I usually print ot an ADL section section G of MDS and ask these 3 questions to Floor nurses: 1. Does resident have a Pressure Sore, Describe Stage and Treatment. 2. have there been any behaviors Sx identified. describe. 3. Ss the Resident in a Toileting program or ADL restorative program: describe. then we go from there to see if a Purpose code R can be done to change level of care. each wekk we review goals set by therapy and approaches met by restorative services. Seems to be going pretty well. If you send me your fax number I will fax the Assessment response sheet for the 211 MESAV.

Can anyone share information with me on how you track/monitor your TILE
levels (texas medicaid) and not miss sending in an "R"?
