if they are able to make own decisions, I care plan & rap this as a STRENGTH, and care plan indicates supporting that independence. It IS a gray area - but whatever they can decide on their own is listed individually (unless they are truly able to make ALL choices) and then monitored for changes as indicated. If it triggers because of a weakness, list that, why it is a weakness and formulate the care plan to provide support for client, and any methods used to promote acceptance (say, of a roommate they don't care for) or to increase independence.
