i came from a facility where we did "unavoidable " on most every resident with pvd disease etc. just recently found out from a nurse who works with utilization review that it is not a good idea to use those forms. on the bottom of the form it states (and you need to check off one) wether it was unavoidable or not. she said that this could get the facility into trouble.
