When someone is readmitted to part A from temporary hospital stay, what type of paperwork do you all redo? dietary, activities, social service, etc admission forms/assessments. Or do you all get copies from previous chart and put it into the new chart (we get new charts for all readmissions no matter how long in hosp.) with a note indicating no changes. Up to this point we have been redoing everything, including an admission MDS. But thanks to your great insight last week, we will be doing it correctly from now on - no raps unless a sig chg with readmit.
