We have pressure risk listed on our transfer sheet and it includes the treatment being
used at the time of transfer, if any. It is listed as SKIN/WOUND CONDITION and if they
have a pressure sore, we send a copy of the skin assessment/treatment sheet to the
hospital with the patient
