Mark, what an
excellent summary it brings back vivid memories of all the interesting
sessions. I still have some notes to transcribe and I hope to
finish them as I shake this flu that I came down with on Thursday
night, that's what happens when you travel in the flu season ;>)! I
missed the boat on taking Amatadine because it caught me by
surprise.
Wayne, you were
asking about the cycling trial. We did meet with Tessa and the trial will
start very soon, she actually signed up her first patient just before ASH.
The requirements are that you have to be in chronic phase and Q PCR
detectable. If I am not mistaken she mentioned to us that she had a few
other patients lined up for the study she mentioned that one or two of them had
a pre-trial treatment of G-CSF and they went to "undetectable" so they were no
longer eligible for the trial- they were bummed" but it does make me think of
good things. I took a picture of her poster and I hope to post it
here.
The premise of her
study: In CML quiescent stem cells persist and accumulate following
exposure to IM, this shows an anti proliferative effect of IM. Basically
her lab work has shown that G-CSF receptor transcript levels are significantly
elevated in CML cells, more so than normal stem cells which suggests that CML
stem cells may show enhanced responsiveness to G-CSF. Her study will
attempt to show if it is possible to reverse the quiescent cells and restore
stem cell sensitivity to IM by cycling G-CSF and IM.
One of the questions
I asked her was - how do you know you are getting the quiescent cells. She
said first you must have your CD34+ cells counted and any lab can do this.
Based on her work she says that quiescent cells are about 1% of the CD34+ cells
(stem cells).
I seem to think that
the patients will be taken off IM for about 7 days, then pulsed with G-CSF then
after about 24 hours IM is re-started. Her lab work showed significant
results in restoring IM sensitive to these stem cells.
