Dear Sheila, You have been given very good advice by both Trey and Christine. Stick to 400mg a day Gleevec and boost up ANC with Neupogen if necessary. Keep track of the platelet counts, it is the trend in the platelet counts that will indicate the way forward. One thing to note is that a FISH test or cytogenetics is the best way to see if your disease is under control. So, I would do the FISH at 3 months interval. You do have to be on the correct dose for some time to see benefits. Another point of note, I do know 2 patients who are 0%Ph+ but with slightly elevated platelet count, one had it transiently, the other still has high platelet count but in CCR. Her onc prescribed her Agrylin to control the platelet counts but what she is doing is looking around for a CML specialist. In your case, Sheila, please coordinate your care with Mayo Clinic and communicate to them your worries. For example, if after some time, the platelet counts still remain high or go on increasing, fax them the bloodwork and ask their advice.
