Hi Drew,
        There are several conditions we check before a page is published.  It must
meet at least one of the following:

        Approved (check box available to a select group of users per page)
        After Date Publish
        Before Date Archive
        Content has changed
        Template has changed

        This greatly optimizes publication time on large sites.  Most sites are
published during early morning cron jobs so it isn't really that important
for many of our clients but for sites that need regular updates, it
certainly helps.

        There is also an optional force all check box at publish time that let's
the user force publications regardless of content/template changes (it will
always respect the others).  And I also offer publishing on certain sections
of the site.  A selected page and all of its children.

        Approval will likely have a pseudo "workflow" routine attached that lets
the user define a series of approvals but we're not there yet.  Whether or
not it should be part of the system has become a heated debate with the
team.  ;)
