I added the missing INSERT|OVERLAY to tack on the original lines, and
ran it against one release of the MVS BCP source code and accumulated
fixes. Out of about 42k files I came up with 20 files with one pair
each.  One of those pairs is strikingly similar:

  EDCPNAME localtime_r(),OINDEX=5E3
  EDCPNAME inet6_opt_next(),OINDEX=965

(each padded with blanks to 80 bytes).  That one also happens to be in
the smallest file that had a pair, at 3001 lines.

Using GETFILES, and LOOKUP AUTOADD instead of SORT, I jammed as much of
the whole mess as I could into storage at once.  Out of about 4.8M
lines, I got about 2700 pairs and no triplets.  Discarding the original
contents of the lines to cram even more in, out of about 7.7M lines I
got about 7k pairs and 5 triplets.

I'll boil the details down some and send them to you.
